Transition Team Annual Health Check

The transition team aims to ensure that young people aged 14-19 who have disabilities/learning difficulties but are not in receipt of social care services receive an annual health check.

We work with the learning disabiities liason nurse to make sure GP surgeries are able to offer the annual health check and that they will inform eligible teenagers of the service before or around their 14th birthday.
