Linking people to health and wellbeing services in the Square Mile
The City Connections Service provides community support assessments for residents aged 18+ and young carers. Through these it aims to understand the client needs and to link them to the appropriate services.
The City Connections Carers' Service includes a combination of focused one-to-one support and online group activities. These are:
- Signposting and Referral Services – we provide one-to-one support to clients to understand their needs and signpost or refer to health and wellbeing services which may be suitable for the individual.
- Volunteer Services - clients can be matched to trained and vetted volunteers, for either weekly telephone befriending calls or weekly one-to-one Digital Buddy IT help and training via an online platform. We are also able to refer clients to volunteer for each role if they are interested.
- Community Connectors - volunteers link up with clients who have a community support plan in order to help them access it. Please note that, due to the Coronavirus pandemic, this service is not currently available as it involves physical interaction.
